Left-handed fountain pen nib grind angles: 0.5 mm italic vs oblique medium
Underwriting vs hooking changes your attack angle by roughly 35 degrees. I put both nibs under a microscope after a month of daily use. The oblique medium wore evenly for me (hooker); the italic caught on the upstroke.
If you push the pen rather than pull it, get the nib ground for it.

This matches my experience — the italic is unforgiving if you rotate the pen at all.
